Former TV star Heather Locklear has been placed on a 5150 psychiatric hold, her second this year, TMZ and other media reported Monday.

The “Melrose Place” and “TJ Hooker” actress, 57, was hospitalized Sunday, Nov. 18, after her therapist and attorney reported that she needed help. An ambulance was dispatched via 911 to her Thousand Oaks home, and Locklear was taken to a hospital, TMZ reported.

While initial reports indicated that she assaulted boyfriend Chris Heisser, TMZ reported that he had not been at the home for days.

Law enforcement is not involved, and medical personnel were summoned to Locklear’s home as a precaution, the celebrity website said.

The star, whose many credits include “Spin City” and “Dynasty,” was previously arrested in June of this year for allegedly attacking a police officer and an emergency medical technician, TMZ reported at the time.

Locklear voluntarily checked herself into a hospital after that incident for alcohol-abuse and mental-health treatment.

The week before the June incident, she was placed on an involuntary psychiatric hold after allegedly attacking her parents and threatening to kill herself, TMZ reported.

She was arrested in February of this year on felony domestic violence charges and battery on a officer after allegedly attacking her boyfriend and a cop.

While under the influence of alcohol, she reportedly nearly bit off Heisser’s nose in the incident, the Daily Mail reported.

Locklear was also hospitalized in January 2012 after her sister reported that the actress had taken prescription drugs and alcohol, according to People Magazine. She was released the next day.

She was arrested on suspicion of DUI in September 2008, and pleaded no contest to reckless driving.

The star’s Thousand Oaks home has been declared a “hazard,” by the Ventura County Fire Department due to the number of incidents and calls to the property, the Daily Mail reported.

Locklear was married to Motley Crue drummer Tommy Lee from 1986 to 1993.

She married Bon Jovi guitarist Richie Sambora in 1994. Their daughter Ava, was born in 1997. The couple divorced in 2007.
